Re: [w3c/ServiceWorker] consider fetching service worker scripts with no-cache by default (#893)

I think, during the f2f, we thought https://github.com/w3c/ServiceWorker/issues/893#issuecomment-248252956 is what devs would expect. (i.e. defaulting to `"no-cache"`.) But with the QPS concern and both (1) and (3) considered together, I suggest we discuss and settle on the right default behavior this time before it's too late.
