Re: [fetch] Mechanism to indicate "destination context" (#64)

I think what we want here is something that matches the first bullet in https://github.com/whatwg/fetch/issues/93#issuecomment-125289983
I.e. I think we should fix the current RequestContext enum to make it indicate the first bullet of that post. That should make that enum useful here too as far as I can tell.
