This replaces @tidoust 's #3231. Thanks @tidoust for creating that one. Only some minor merge-conflict resolution was done here vs yours, with a couple reorderings of the checks of mediaSource.duration to come after endOfStream() (shouldn't have any effect, but probably is less fragile this way).

I'll merge this shortly. Note this VPQ feature is not in MSE v1, but it may help implementors and especially web authors to understand the originally intended behavior of that feature.
