ACTION: Complete availability survey for ACT TF meetings

I have updated the availability survey for the ACT TF meetings so we can
know who can participate in calls for the next few months. This is
especially useful to determine what topics to discuss during the various
meetings and if we'll have enough participation to hold meetings during the
holidays.
