Bugs in the form generating DB entries

Hi Shadi,

I detected two bugs in the form generating DB entries:
- The RDF file is broken: the element <dct:hasVersion 
rdf:parseType="Literal"> is not closed properly.
- Some elements in the Other: part of an input field, do not appear in 
the RDF.
