I took a stab at addressing #1211-#1213, aka the applicability of
tts:direction to <p>:

The proposal is an attempt at compromise:

- it maintains the spirit of XSL:FO: tts:direction does not affect the IPD
- it is compatible with CSS: the paragraph embedding level is set by
the IPD as computed on <region>
- it minimizes changes to the specification
