In section 7.4 is

#extent-region  permitted       When applied to a region element, the 
tts:extent attribute shall use px units or "percentage" 
representation, and shall not use em units.

however, it is technically possible to use 'em' on extent (and origin)
 in the image profile, which does not specify this constraint.

Should move this constraint to section 6.10.
