Purposes and DNT extension Addendum

Here is the document I was asked to generate in the last call,  extracting
the purposes extensions into its own document.

I have added some more descriptive text and responded to concerns expressed
about fingerprinting by limiting the API to a site-specific call, 

when the API call is made within a user gesture, and the browsing context is
secure.
