Issue 22 - Inputs received

Hi Folks,

the enclosed PDF should contain a consolidated version of all inputs
that we received (via list and tool). Please double check for
completeness: If there was anything posted in time, it should be on the
sheet (for Walter, I received two inputs and I used the revised second one).
