Issue cleanup and sync for TPE document

 Hi Team,



a while ago, David has identified a series of inconsistencies between
the TPE document and the issue database.
I tried to resolve these issues while cleaning up the status of the
issues on TPE.


Substantial changes that I made were:

ISSUE-83 (How to withdraw out of band exceptions)- changed to CLOSED: We
agreed that a management URL can be published via the well-known URI.


ISSUE-113, ISSUE-129, ISSUE-130: I closed those issues since we agreed
in Bellevue that there should be (at least) a web-wide and site-wide
exception.


ISSUE-146 (Maintainability of responses at well-known URI): I closed
this issue since we agreed that a combination of w.k. URI and response
headers should be possible.
