Maybe the fact that the form requires login is a deterrant for some of us?

Note that the spreadsheet does not record any login information,
neither your name nor email etc (spreadsheet does not tell who filled
out the form AFAIK other than the info you enter)

would be nice to have a bit more input, thanks a lot for your
