AS2 issue handling

Hi all,

Due to the imminent transition request for AS2, we're making sure all of
the closed issues have been given due attention. I've been through and
labelled them all as either 'commenter satisfied' or 'waiting for
commenter', and I'll be contacting everyone who raised an issue on AS2 and
didn't indicate whether they were satisfied with how the issue was handled
to check if there is further action needed (and ccing this list, stand by
for small email surge).

The process I used to decide whether to label an issue as 'commenter
satisfied' or 'waiting for commenter' is here

If you notice I've labelled an issue you raised as 'satisfied' and you feel
this is in error, please let me know. Similarly, if you see yourself on the
list on the wiki before you get an email from me and are happy for the
issue to remain closed, leave a note on the issue and add the date to the
'resolved' column on the wiki.
