In section 2.8 of the current draft:

(and, for that matter, the CR version:

there's a reference to an m:MaxTime element in the example titled:
"Example: SOAP 1.2 Fault payload with the contentTypeMismatch subcode"

However there's no namespace prefix binding declared for "m", so this
breaks the example.

Proposal:
Since the "Detail" element is optional in SOAP 1.2 [1], and the "detail"
adds nothing to the example in question, I recommend just dropping the
env:Detail element and its contents from the example in question.
