SURVEY on Decision Policy, closes Tuesday June 20th before meeting

Per our discussion at last week's meeting, we've created a survey regarding switching to the W3C decision policy<https://www.w3.org/2002/09/wbs/35422/w3c_decision_policy/>.

There is a lot of information, so we've attempted to present it two different ways.
