I updated the pull request as planned. This should also fix #281 "PresentationReceiver.connectionList doesn't return the same object all the time". As explained in the commit message, I introduced two new idioms: "presentation controllers monitor" (the `PresentationConnectionList` object) and "presentation controllers promise" (the `Promise` to get that object) to make it easy to reference these concepts in the spec. It may be possible to find better names.
