CAPTCHA update

Dear colleagues,
As can be ascertained from the list of open issues on CAPTCHA:

  *   A couple of open issues on the document were in the issue list of the main APA repository. I’ve transferred them to the captcha-accessibility repository.
  *   I’ve created a Pull Request to address issue #17 with two editorial changes.
  *   I’ve added issue #19, based on the discussion at an RQTF meeting of voice recognition used for biometric purposes. I have also created a pull request to add this example to the document.
