RE: [CAPTCHA] Document restructuring

I reorganized the document in the manner proposed at this week’s meeting. I also made minimal changes to the content (mostly modifying introductory text to reflect the changed structure, but also clarifying the interactive nature of the Cryptographic Attestation of Personhood technique).

  *   Did I accidentally delete anything that I shouldn’t have deleted?
  *   Am I drawing the right distinctions here?
  *   What changes, if any, should be made to the classification of techniques proposed in the revised structure?
  *   Friday afternoon writing is unlikely to be my best. How should the introductory text be improved?

Let’s review this work and decide whether it should be merged.
