Better document structure for XAUR

Hi all,

I've changed the layout of the XAUR document based on input from 
Michael. It was super useful.

The structure is better as related items are better grouped and the 
sections such as 'Understanding XR and accessibility challenges' and 
'User Needs and Requirements' are clearer.
