Draft CAPTCHA notes updated

I've updated the notes on CAPTCHA, integrating Scott's contributions from last week. I still have some papers to summarize and to add to the references.

If I and anyone else with contributions still to make could finalize them over the next week, we can then review, edit and notify the APA Working Group.
