incident resolution moved

Hi All,

I created a pull request in w3.org/guide [1] for further discussion of 
incident resolution and ombuds. Please remember that this does not have 
to be perfect. We will work with the ombuds organization that is coming 
on board in a few weeks to refine this. We are getting this to a stage 
of "Are you comfortable posting this so that we can have a policy that 
no longer points to old information?".

I see some comments in the google doc around issues like anonymity. 
Christine has reviewed this document, but I will ask her to take another 
look.
