revised ombuds and procedurs document

Hi PWE,

I have been working with Catrina, Christine, Wendy, and Sheila to update 
the Procedures document to include information about ombuds with 
tEQuitable. I am sharing a draft

in Google Docs. This is a merger of the following:

* the ombuds doc we put on w3.org/guide (I made some changes in 
"suggestion mode" so you can see what's different)

* our incident resolution draft

* Catrina and Christine's document about raising a formal complaint

* tEQuitable's explanation of their services

There are a few open questions, some that specifically say "PWE" because 
we want your input.

I am sharing this a week before our next meeting, because we would 
really appreciate your thoughtful input. The agenda next week will be to 
discuss this.
