Additional TOC issues

Just a ping to all interested: now that the TOC structure and extraction algorithm is part of the latest WP draft, I have added some very specific, somewhat more technical issues that came up during the editorial work. These are:

- #376, whether we would allow creating a TOC directly into the manifest (as agreed on the last telco, this was re-introduced as a separate issue while closing the rest
- #377, whether we should consider audio, video, img, and object as a possible 'link' in a TOC
- #378, what elements should be ignored during extraction
- #379, minor editorial questions
