XML Update (30 March)

Hi all, some updates to the XML Encoding [1]
- fixed XML namespace (missing end /)
- added id/idrefs (section 3) description of relevant elements and cardinalities
- updated wording in section 4.1
- fixed a few anyURIs to URIQnameQcode in the XML Schema
