General music notation

Hi all,

I've just pushed a new version of the MNX proposal. This contains
significant new material describing a general approach to music encoding
called "GMNX". This complements the CWMN-centered approach that was mostly
discussed in Frankfurt (now called "CWMNX" for clarity).

Other material has also been added.

Next up: migrating all known MNX issues into to Github. I will send out a
