I'm proposing: Maturity-Assessment.xlsx And, to answer Stacey's question, no, we don't need to say this is Excel. The file extension makes that clear. Furthermore, other tools (like Linux/Unix LibreOffice and Apple's Numbers) are perfectly able to read and write .xlsx files.
