some reading for the meeting

I am scheduled for the agenda for the upcoming meeting to speak on AT needs
to clarify speech in MathML. I started to write some slides and then
decided to make it a document that someone can read if they aren't part of
the meeting. I'm sure that there are plenty of typos in it and I'm also
sure that I could have said more and included more examples, but it is
plenty long as it stands. I committed the document to github:

For those of you with github access, feel free to clean up any typos or bad
grammar. No need to ask me for permission...
