I checked in a version (a copy of the .tex .md document) that has auto-generated table of contents and mathjax rendered math https://mathml-refresh.github.io/mathml/docs/accessibilitymj there are now three versions of the text checked in (accessibility.md, accessibility.tex.md and accessibilitymj.md) which seems a bit unstable, if people are Ok with the mathjax version we could lose the first two and rename this to accessibilty.md ?
