Version 2.0.0 of the NineML tools are (mostly) published

Hello,

Over the spring holiday, I tidied up and published version 2.0.0 of
CoffeeGrinder, CoffeeFilter, CoffeePot, and CoffeeSacks. I ran out of
time before work started this morning to finish up on CoffeePress, but
I’m planning to work on that this evening/soon.

These are mostly the same as the 1.99.x versions that have been in
testing for months, except:

1. I fixed a bug where hex encoded characters from the astral plane were
   mangled.

2. I rewrote the Saxon extension function API so that there’s a single
   function to load a grammar and it returns a function that will parse
   inputs with that grammar.
