Open issue: simple namespaces proposal

I think we should adopt this proposal. Setting aside the question of
whether or not users *really* need namespaces in ixml, they’re going to
expect namespaces in an XML specification. I have been persuaded that if
we don’t do it in 1.0, we will be pressured to do it in V.next and when
we cave to that pressure, we’ll do exactly what this proposal suggests,
so we might as well just do it now.

I am strongly in favor of adding a single default namespace declaration,
but I’m in favor of accepting the proposal even without it.
