Modularisation proposal

As Ada mentioned, we wanted to get an initial proposal out to the group to
start discussion on the modularisation in tomorrow's call (although we
expect to continue discussion in next week's call too).

I just posted the initial proposal from the Chairs and Editors in
https://github.com/immersive-web/administrivia/blob/master/modules.md.
