PKCS #11 standard in progress

Locating and enumerating PKCS #11 providers has always been a hassle
compared to using Microsoft's CSP.

This should have been addressed years ago by the responsible SDO (RSA),
but they didn't.   The faster and (often) better alternative emerged anyway:

code + spec = standard

Token provisioning is still not covered by CSPs and PKCS #11 so in this
case there will a brand new "companion" API for this very purpose.
