I have fixed an additional instance with "???". I also finally think I understand the use of the various 'ref' elements: <specref>: internal reference for general purposes <xspecref>: cross-reference to other document <bibref>: reference to bibliography entry The whole thing resulted in the addition of UTR15 to the references section; it was referenced, but only inline in the text.
