Tutorial design change

I uploaded this morning the changes to the first tutorial, as discussed on last night's telecon.

The default page is now the all-in-one version.

This allows users to jump to a specific point in the tutorial, but still manages content negotiation for language as before.  The index page is still there, but is not the default.  I also separated the fragment ids for sections/slides in the all-in-one from the slide numbers.  This means that we can change or rearrange the contents without the fragment ids breaking (as would have been the case before, since the relevant ids were automatically generated on the basis of the slide order).

I also updated the template used for the all-in-one page to the latest version, including the addition of a feedback form.  I'm hoping we'll get some responses to show that people using the validator went from that via O-charset to this.

I will work on other tutorials as time allows.
