Thanks for bringing this up.  I'm not sure the XHTML guys will want to specify *how* this is achieved, leaving that for implementations, but it may be appropriate to describe the expected behaviour.

Do you mean because you can't add markup to the title element?  (Note that in XHTML 2 you will be able to (since we asked for it)).
