DRAFT minutes of FTF

Thanks to Peter for pulling together the minutes of our FTF.  Please
review the draft version enclosed and send any comments to the list so
that I can publish the final version to the site next week.  (I have
made some changes to Peter's notes at the beginning, and added the toc
we created).
