I think it is a rare case, it is difficult for a document as _clreq_ to give instructions to every extreme cases, where house rules should be applied. e.g. the case shown in the pic below. For automatic type-setting, this is tolerable, but In printing matters, designer usually may avoid pinyin and emphasis marks to appear on the same side of base characters.
