I noticed the ontology mapping project members recently proposed <https://github.com/mapping-commons/SSSOM/issues/48> using the eco <https://www.ebi.ac.uk/ols/ontologies/eco> ontology to document explanations why an equivalency or other mapping was made. I think the eco ontology is general enough to be used for human services, but I may be wrong, since it is heavily populated with biosciences evidence terminology. In which case, patterning something more general from eco would be useful. Regardless, documenting the rationale for a given mapping would assist future human services ontology maintainers assess the continued validity of those mapping.
