Change Description: <menu> item removed from CR
Rationale:

As part of Robin's recent CR updates, the <menu> element has been removed
(since we needed to remove <command> and <menu>, <menuitem> made little sense
without it. As such, you could say that the differences with the menu item have
been resolved.

I'm choosing "Fixed" for this, but if this feels like "Won't Fix" I'd accept
that too.
