[Bug 11731] Replace <hgroup> element with a <subhead> element used as the child of the <hx> element

Krijn suggested using <small>. Advantage is that the default styling is closer
and we do not need a new element. Disadvantage is that the meaning of <small>
depends on the context, but I am not sure that is a problem. (You could even
argue that <small> serves the purpose of clarifying text.)
