"The initial offer must include": Add RFC4733's "named telephone events" audio
codec to the first audio stream or add an audio stream with just that codec if
there isn't one already. Add that codec to all audio streams added with
addStream(). Add an API that sends DTMF events down the first stream with that
audio codec (if any), for 0-9, *, #, A-D, with a fixed arbitrary volume and
two options for duration, short and long.
