[Bug 10366] please ban <input type=file> if the form's enctype is application/x-www-form-urlencoded, as this seems an authoring error

Change Description: no spec change
Rationale: You can use <input type=file> without a form at all, via script. It
would be unfortunate to require that such an input avoid being put in an
unrelated form, even for just a moment (e.g. while providing the user with some
UI to fill in other controls from a file or some such).
