ben, the sorting, selecting and suppression of repeated text/items that 
fulfill an identical purpose is something that should be left to the user's
assistive technology -- most ATs have mechanisms for filtering out repeated
text, redundant hyperlinks, etc.

it is best for the author to do ALL she can do to provide contextual and
meaningful information about the objects in her document -- as someone totally
dependent on speech-output, i find that a bit of redundancy is a small price to
pay to ensure that the content is accessible not only to those who own the
"latest and greatest" (and most expensive) screen readers, but to those who
have no choice but to soldier on with older versions of AT, a significant
factor given the economic realities that face persons with disabilities -- one
of MY primary backwards-compatibility issues...

FWIIW, to me, the logical default role for IMG is role="img" -- i believe that
any IMG without a role should be treated as if role="img"
