OK, Gez you are right, that was a frustrated and caustic message; You are also right; people have concerns about this area. In fact, we had a workshop on it before TPAC. The concerns are shared. Bugs are a good tool for some purposes; however, not everything is a nail. Bugs are very good for tracking single-issue questions where the resolution is either clear, or the editor can work it out. Bugs are a poor way to a) have a conversation about a problem area; b) get a grip on understanding a problem with multiple facets, for which the resolution may well involve a number of discrete changes. I think that accessibility of multimedia needs a holistic look and a conversation, and so I find filing bugs on it not helpful, and asking when "Later" is could be answered "when there are consensus recommendations as to what to do about it".
