repeat index change

Hello,

it's not the first time that we were asked for this on our mailinglist 
so i thought i report it here.

There is no way in XForms to detect a index change in a repeat - 
probably some focus magic can do it but that's not at all intuitive.

A 'xforms-index-changed' or something like that would be quite helpful.
