Completed action to update spec and test suite tests for Appendices B9, B11, and B14

The following action item was completed:

Due to the change to using setvalue for attribute replacement, the 
appendix was renamed "Data Mutation Patterns".  The ID space of the 
Appendix and its subsections was similarly renamed.

The test suite tests were changed to match.  The test suite zip file was 
not updated since other test suite changes are expected.
