DID specification FPWD draft ready for review

Hi all,

Per the DID WG call today, a draft of an FPWD document with an intent to
request to publish the draft during the DID WG call next week (in 7 days).

This PR readies a FPWD for the DID specification by

* Changing the render template to FPWD.
* Adding all current issues to an appendix in the spec.
* Updating the Status of the Document language to warn implementers not
  to implement and point to all issues and PRs in the appendix that may
  result in changes to the specification.

It does not yet (but will before FPWD):

* Update the shortname of the specification.
* Update to the latest list of issues and PRs.
* Update the links to the latest shortname in Github.
* Create a static copy of the FPWD document.

As Dan mentioned on the call today, please review and mention concrete
changes to this document in order for you to not object to it being
published as an FPWD. As a reminder, an FPWD does not mean that you
agree with everything in the specification and many more months of work
is expected before we get WG consensus on the document. Please don't
wait until the last second to review/object.
