The original question (layout of the ::v-t pseudo in relation to the scope) has been addressed by the Nov 2025 TPAC resolution. Specifically, the ::v-t pseudo is a box-tree child of the scope, but paints as a separate stacking context overlaid on the scope that is outside the scope's overflow clip, scroll offset translation, and effects such as opacity. The explainer, spec, and implementation (in Chrome) have now been updated to reflect this. Hence I think this issue can be closed.
