In my opinion, `-outset` has the same drawback as `-inset`, that it sounds like it could be affecting layout and not merely rendering; but in addition, it would have us using negative values for the common case, at least in the case of text-decoration (I'm not sure which way the balance is likely to lie for column-rule). I also think "outset" is less familiar in general as an everyday word/concept than "inset".
