Gotcha, that makes sense - this would require just a chunk of extra bookkeeping. Essentially, if there is any item with alignment in the masonry axis, we'd need to keep track of all gaps created by spanners (similar to dense packing), as well as the last item placed in each stack above the spanner that is associated with that gap. And then apply alignment to those potential items at the end if applicable.
