OK, I've been thinking about this... I *think* the right answer is to lay it out using block-start/inline-start, but transform it using physical coordinates (at least for now, since we don't have logical transforms). So if the author *turns off* the UA-provided transforms, the element is anchored to the start/start corner even on an RTL page.
